I understand and acknowledge that I Kicked Sugar™ (IKS) MasterClass is an approach to health and wellness and is not a medical, nutrition, or diet program, and that Joyce Laszloffy is not a licensed health professionals and has no healthcare training.
I agree to seek the clearance of a physician or qualified medical professional to manage my health and determine if I Kicked Sugar™ (IKS) MasterClass and its components are appropriate for me.
I understand that my success participating in I Kicked Sugar™ (IKS) MasterClass depends upon my commitment. I am ready, willing, and able to devote the time needed to complete and fulfill the Course. I agree to be responsive and cooperate in meeting the ongoing requirements of participation inI Kicked Sugar™ (IKS) MasterClass in a timely manner.
I understand that I Kicked Sugar™ (IKS) MasterClass takes no responsibility for managing my health and is not serving as my doctor, dietician, or other health professional role.
I understand I may be required to download and use (at no additional cost) 3rd party tools or applications necessary to receive the Services I Kicked Sugar™ (IKS) MasterClass . All such tools or applications are provided by the 3rd party(s) under their own terms of service. II Kicked Sugar™ (IKS) MasterClass is not a party to such agreements and does not endorse, warranty or assume any liability for any 3rd party tools or applications.
TERMS + CONDITIONS & RELEASE WAIVER
I hereby grant permission to Joyce Laszloffy DBA I Kicked Sugar™, healthy education, LLC to use my photograph and/or any testimonial/review I give regarding the I Kicked Sugar™ (IKS) MasterClass and or Monthly IKS Rise Up Membership in any print and electronic form including, but not limited to, marketing, contests, advertising and educational materials used to advertise the I Kicked Sugar™ Course, including use on the I Kicked Sugar™ website and social media pages. I acknowledge the right of I Kicked Sugar™ to crop or otherwise edit any photographs or testimonials/reviews at their discretion. I also acknowledge that I Kicked Sugar™ may choose not to use my photograph and testimonial/review at this time, but may do so at their own discretion at a later date. I also understand once my image is posted or submitted on the I Kicked Sugar™ website and social media pages, the image can be downloaded by any computer user, which is beyond the control of I Kicked Sugar™ and I will hold them and any of their affiliated offices and vendors harmless from any such use or download. I understand the policy of I Kicked Sugar™ to only use first name and last initial when labeling photographs and testimonials.
Authorization and Release Information
I understand my testimonial as outlined above (the "Testimonial") and made on behalf of I Kicked Sugar™ (hereinafter called "The Company") may be used in connection with publicizing and promoting The Company. I authorize The Company to use my name (First name and First Initial of Last Name), brief biographical information, and the Testimonial as defined on this form.
I hereby irrevocably authorize I Kicked Sugar™ to copy, exhibit, publish or distribute the Testimonial for purposes of publicizing TheI Kicked Sugar™ Course or for any other lawful purpose. These statements may be used in printed publications, multimedia presentations, on websites or in any other distribution media. I agree that I will make no monetary or other claim against The Company for the use of the statement.
In addition, I waive any right to inspect or approve the finished product, including written copy, wherein my likeness or my testimonial appears.
I hereby hold harmless and releaseI Kicked Sugar,™ Healthy Education, LLC from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
I have read and agree to ALL Terms and Conditions as stated here and on the Company website as well as the DISCLAIMERS. I agree to the authorization and release of information and give my consent for the use as indicated above and release Joyce Laszloffy DBA I Kicked Sugar™, Healthy Education, LLC of any liability by clicking the box YES on the Accountability Form.
